Connecting patients and doctors to better manage chronic disease

Aster DM Healthcare won Gold for Innovations in Healthcare Technology at the 2017 Asian Hospital Management Awards.

With the objective of helping patients with chronic diseases live healthier, longer and quality lives, Aster DM Healthcare from Dubai, UAE launched the Aster Chronic Care@Home programme – an Intelligent Health Service Platform that combines the concept of constant connectivity, advanced smart-device technology, remote chronic care management from home and continuum of care.

The platform connects their vast network of practitioners and clinics to their patients at home to assist in monitoring and managing readings such as blood pressure levels. Readings are then automatically updated on the patients’ smartphones through a mobile app to provide updates to paramedics via contact centre.

Patients and caregivers will be contacted by the group’s care specialists if there are any fluctuations in the readings by the healthcare group’s care specialists.

Aster DM Healthcare won the Gold Award for the Innovations in Healthcare Technology Project category during the Asian Hospital Management Awards in 2017. This award category recognises best practices for improving the quality, safety and efficiency of healthcare delivery with the use of technology.

The Asian Hospital Management Awards, now in its 18th year, is a prestigious ceremony which recognises and honours hospitals in the Asia Pacific for best practices to improve healthcare.
